• Request High School or GED Transcripts

• Request previous college transcripts and evaluations

• Achieve minimum scores on the college placement test

• Submit completed Health Science application

• Submit completed freedom from narcotics statement

• Submit a copy of your valid Florida driver's license

• Have an interview with the program manager

• A criminal background check is required after provisional acceptance click the link to fill out the form

• A physical exam is required upon acceptance to the program
